Which questions appear before each vitality capacity test?

The questions at the beginning of the vitality capacity test originate from the Multidimensional Fatigue Inventory (MFI-20). Only the questions for the physical fatigue domain are requested to be filled in. 

The answers to these questions are used to calculate the person’s self-perceived fatigue (physical fatigue) and vitality capacity (Capacity‑to‑Perceived Vitality ratio = CPV-ratio).
